Physician Led Specialty Clinics
Perth Family Health Team (PFHT) offers a range of specialized clinics to support the health and well-being of people in our community. These referral-based clinics provide focused assessment, care and expertise in areas such as memory and cognitive health, healthy aging, wound care, and psychiatry.

Our specialty clinics work collaboratively with individual primary care providers to complement existing services, support patient care needs and help connect patients with the specialized expertise they require.
Healthy Aging Clinic
The Healthy Aging Clinic supports adults and their caregivers, who may be experiencing cognitive or functional changes and may benefit from specialized assessment and treatment planning. The clinic accepts referrals for concerns such as memory changes, cognitive decline associated with chronic disease, management of multiple health conditions, polypharmacy, changes in mobility or daily functioning, and assessment of medical fitness to drive.

Working collaboratively with patient’s primary care provider, the clinic aims to support early identification of concerns, cognitive assessments, optimize health and functioning, and inform care planning to help adults maintain their independence, safety and quality of life as they age.

MINT Memory Clinic
The Mint Memory Clinic is an interdisciplinary evidence-based approach to supporting patients experiencing memory concerns and their caregivers. From the first referral the Mint Memory Clinic works in partnership with your primary care provider to develop proactive care plans and make key referrals to specialists.

Through this clinic, patients and their caregivers are supported by a specially trained physician, registered practical nurse, occupational therapist and The Alzheimer's Society of Lanark Leeds and Grenville. The Mint Memory Clinic is conducted in 2 separate appointments:
​
1. In home (preferrable) visit (approx. 1-2 hours) to conduct memory testing and activities of daily living risk assessment with RPN and OT.
2. Office appointment (approx. 1 hour) conducted by physician, RPN and meet with the Alzheimer's Society.

Wound Care Clinic
The Perth Family Health Team (PFHT) Wound Care Clinic provides specialized wound care using best practices and evidence-based approaches to support wound healing and improve patient outcomes. Working in collaboration with patients and their primary care providers, the clinic supports the assessment and management of specific wound types, including diabetic foot ulcers, surgical wounds and traumatic wounds. The clinic aims to promote healing through individualized care planning and coordinated treatment recommendations tailored to each patient's needs.
Wound Care Clinic requires a referral from a primary care provider, hospital-based care providers and specialists.
