Older Adults
People with complex health conditions, including frailty and dementia can benefit from the multi-disciplinary primary care geriatric clinics which focus on the health and well-being of older adults in our community.
The clinic offers a range of services to help identify, assess, and treat various health conditions that affect older adults such as: early screening and diagnosis, care planning, and treatment.
You can also get help from these Health Professionals to improve your quality of life:

An on-site social worker is ready to help you move through your various stages of care so you are not trying to figure it out alone. These services require a referral.

MINT Memory Clinic
If you are concerned about memory issues, the Multi-specialty INterprofessional Team (MINT) provides high-quality dementia care and support.
It is the goal of the MINT program to help older adults stay home longer and out of hospitals and long-term care.
This standardized and nationally accredited program includes specially trained health professionals with more than ten years of experience. The MINT model works within primary care networks to offer access to high-quality dementia care and support in the community. These services require a referral.
