Allied Health for
Older Adults
People with complex health conditions, including frailty and dementia can benefit from the multi-disciplinary primary care geriatric clinics which focus on the health and well-being of older adults in our community.
The clinic offers a range of services to help identify, assess, and treat various health conditions that affect older adults such as: early screening and diagnosis, care planning, and treatment. ​

You can also get help from these Health Professionals to improve your quality of life:

An on-site social worker is ready to help you move through your various stages of care so you are not trying to figure it out alone. These services require a referral.

MINT Memory Clinic
If you are concerned about memory issues, the Multi-specialty INterprofessional Team (MINT) provides high-quality dementia care and support.
It is the goal of the MINT program to help older adults stay home longer and out of hospitals and long-term care. ​

This standardized and nationally accredited program includes specially trained health professionals with more than ten years of experience. ​The MINT model works within primary care networks to offer access to high-quality dementia care and support in the community. These services require a referral.
