Perth Family Health Team History
Founded in 2024
Funded by Ontario Health
The Perth Family Health Team was founded in May 2024 as part of the a $110M investment by the Ontario Government to add over 400 new primary care providers and 78 new and expanded interprofessional primary care teams across the province.
The Perth Family Health Organization including: the Perth Medical Clinic, Elliot Street Clinic and Perth Family Medicine received $3.1M to support existing primary care to help manage increased operational costs and ensure timely access to services to a range of health care professionals.
Extended Health Care
The Health Team was also tasked with improving access to mental health professionals, addictions specialists, as well as primary care for those without a primary care provider. Other services also include increased support services for older adults, occupational therapy, and Indigenous care navigation.
